Online Self-evaluation Questionnaires

Welcome to the Supporting Families Self-Evaluation webpage.

Below you will find a range of clinically approved, short, self-evaluations, which can help you gain an understanding of your own wellbeing and  decide if you may need to seek professional help. Some are about drug and alcohol use, some about mental health and some about overall health and  well being. 

There is a brief description of each of the evaluation tests, to help you choose what aspect of yourself you would like to find out more about.

All of the evaluations have been designed  for adults, although we have included one for children  -”The Strengths & Difficulties Questionnaire”.  We have included this as it can be a great way to have a conversation with a child/ young person about how they feeling, and managing.

These short questionnaires can only provide you with an indication about your own health and wellbeing, so if you have any concerns, or want more advice we suggest that you contact your GP, or please do give us a call at any of our offices, and we can help you find the services you need.

Just choose the evaluation you’d like to take by clicking on it. Complete the form by clicking the answers that have the best fit for you, enter your email address at the bottom and click “submit”.

Your completed evaluation will be emailed to you at the address you give. You may use that information to start a discussion about your health and support needs with someone else, or do the evaluations regularly and keep an check on your own progress and needs.

Leeds Dependence Questionnaire (also known as LDQ)

This questionnaire measures alcohol and opiate dependence. It asks a series of questions about the importance of alcohol and/or other drugs in your life.

Strengths & Difficulties Questionnaire (also known as SDQ)

This questionnaire focuses on child and adolescent behaviour. It screens for conduct problems, hyperactivity, emotional symptoms, peer problems and pro-social behaviour. It is used widely in New Zealand, especially in Child Adolescent Mental Health Services.

The Kessler (also known as the K-10)

This short questionnaire is a simple test of ‘psychological distress’. It asks basic questions about your level of anxiety and depressive symptoms over the last four weeks. 

The Cannabis Use Disorder Test (also known as CUDIT-R)

This short test helps identify problematic cannabis use in adolescents and adults. It was developed and validated in New Zealand. 

Alcohol Use Disorders Identification Test (also known as AUDIT-C)

This is a brief screening test to help identify hazardous drinkers or those who have active alcohol use disorders (including alcohol abuse or dependence). The test asks about frequency of drinking, quantity consumed and frequency of heavy drinking.

Measure Your Wellbeing

This is a family and whanau focused questionnaire. It assesses the wellbeing of those supporting a family member with mental illness and/or addiction.
